Self Advocacy and Transitions Support Worker

Job title: Transitions and Self Advocacy Support worker                        

Location: Hammersmith and Fulham offices. Also out in the community.

This role would be mainly supporting the work of the Youth and Transition project – Safety Net Youth (SNY) within Hammersmith and Fulham (HF) Mencap. SNY is a transition project for young people aged 13-25 with learning disabilities; focusing on skills development, building confidence, independence and positive aims for the future.

Contract type: Zero hours                                                                                      

Salary: £9.15 per hour

Main duties:

  • To support the work of the Youth and Transition project – Safety Net Youth
  • To support children and young people from the age of 13yrs to 25yrs
  • To work mainly in the school holiday’s and / or in after school sessions
  • To engage members to take part in activities and support their individual needs
  • To encourage members to develop their confidence and independence
  • To encourage social interaction and inclusion and to empower the members to become more independent
  • To liaise with parents and carers both face to face and on the telephone
  • To work as part of a dynamic team within HF Mencap

Qualities and experience needed:

  • Experience of working with people with learning disabilities
  • Must be flexible
  • Must have good communication skills
  • Must be able to work as part of a team
  • Must be able to take initiative
  • Must be able to follow instructions
