The service offers co-ordinated, multi-agency support for families with a child under five, who has a disability or complex health needs, where many professionals are involved.

The main aim is to bring together appropriate services and support and to assign a Key Worker to each family to co-ordinate the different agencies.

The Early Support Programme also provides materials on disabilities and information on local support.

The Key Worker can :

  • Offer advice and information to families
  • Be the main point of contact for other professionals/agencies
  • Provide practical and emotional support
  • Give information on services and agencies
  • Help arrange Family Service Plan meetings and monitor the plan
  • Ensure that families are fully involved in decisions and services provided for their child
  • Coordinate and/or attend meetings with professionals and the family
  • Identify any gaps in service provision and ensure appropriate referrals are made
